Licensed Practical Nurse

Licensed Practical Nurse (LPN) Currently looking for EXPERIENCED Licensed Practical Nurse(s) (LPN) to fill multiple openings with a home care agency located in Raritan, NJ. Interested candidates should have a minimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility. Responsibilities of the Licensed Practical Nurse (LPN)

• Provide thoughtful care to client & patients based on assessment and personalized Plan of Care
• Monitor and track change in the client / patient’s condition and needs, keep the Director of Nursing and any other appropriate personnel informed
• Proactively update Director of Nursing on health changes and concerns
• Pay rate : $24- $35hr., depending on assignment Requirements of the Licensed Practical Nurse (LPN)
• Current unrestricted LPN license in New Jersey
• Pediatric experience preferred
• Current BLS certification
• Negative PPD or chest x-ray
• Minimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility
• Licensed driver with valid automobile insurance
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull.
• Varied assistance is required, depending on the mobility of the client.
• Must be able to lift up to 50 pounds
