What You’ll Do

• As a key member of our clinical team, you’ll direct, coordinate, and deliver comprehensive medical and clinical services to members in our clinic. This includes engagement, assessment, diagnosis, treatment planning, medication management, education, referrals, risk assessment, and discharge planning. You’ll also help lead quality improvement initiatives, support utilization management, and collaborate closely with internal and external care teams.
• You’ll report to the Medical Director of Primary Care and Chief Medical Officer for clinical performance, and the Program Director for administrative oversight.

This is a full-time, onsite position within one of our integrated care clinics. Flexibility to provide clinical coverage at other locations may occasionally be required.
