Job descriptionOne year of proven experience working with adults with developmental disabilities. Candidates must be able to take the initiative, work independently, make executive decisions when necessary, and meet deadlines. Excellent written and verbal communication, reliable car, flexibility to attend meetings when needed during the day. This hybrid position requires field visits; candidates must be flexible to travel within these counties. The state of New Jersey requires Covid vaccination for this position. Must pass criminal background and CARI check. A bachelors degree in psychology, sociology, social work, or related field. Assessing service needs, developing the New Jersey Individualized Service Plan (NJISP) for access to services, and liaison with clients, families, programs, and service providers. To establish achievable goals, outcomes, and service eligibility. Development of a Person-Centered Planning Tool (PCPT) is a mandatory discovery tool used to guide the person-centered planning process and assist in developing the NJISP. Ongoing monthly monitoring of clients. Coordinate and facilitate team meetings off-site at either Residential and / or Day Programs to identify service needs.

The caseload varies from 10 to 20 consumers.
Pay is $1,500 - $3,000 Monthly, commensurate with experience and caseload.
Must reside in the State of New Jersey
