Job descriptionBoard Certification in Internal Medicine, Family Medicine, or Geriatrics (preferred). Active, unrestricted California Physician License, DEA, and CDS certification. Eligible for Medicare and Medi-Cal enrollment. Competitive compensation ($250,000 $280,000 base) plus bonuses and equity. Medical, dental, and vision insurance. Internal Medicine or Family Medicine Physician
Position Overview :
Seeking a Primary Care Physician to join a comprehensive care program focused on seniors. This role involves in-person assessments, care coordination with an Interdisciplinary Team (IDT), and oversight of participant care in various settings.
Key Responsibilities :
- Conduct comprehensive assessments and develop individualized care plans.
- Collaborate with IDT and community physicians for discharge planning and care delivery.
- Oversee care in nursing homes, hospitals, and participant homes.
- Provide direction to Nurse Practitioners and participate in rotating night and weekend call.

5+ years post-residency experience.
