The licensed practical nurse is expected to perform tasks and responsibilities within the framework of nursing services and the scope of practice in a skilled nursing facility. The LPN is part of the interprofessional team to help lead the resident and family teaching program through health teaching, health counseling, provision of supportive and restorative care, under the direction of a registered nurse or licensed clinician / provider.
Provide comprehensive care to patients by applying the nursing process in an organized and systematic manner according to policies of the Medical Center and The Transitional Care Unit in keeping with patient safety principlesPerforms all clinical skills and services as per policy and accepted procedure within the LPN scope of practiceCompletes clinical competency, required certifications, and peer evaluation processCollaborates with the resident, significant others, ancillary departments, and functions as part of the multidisciplinary health care team whose goal is to provide care that is continuous and well-coordinated with specific discharge goalsPerforms effectively and efficiently in emergency situationsEvaluates and documents resident progress / response toward expected outcome(s) and reports pertinent observations and recommendations to the interprofessional teamCommunicates proactively to the interprofessional team the plan of care changes in patient condition, the patient progress, the discharge plan and other relevant informationDocuments resident care and required safety checks accurately and legiblyPerforms ongoing evaluation of residents’ physical and mental health and notifies the interprofessional team and / or clinician / physician, including abnormal lab and diagnostic resultsExplains medications, procedures, and treatment protocols to residentsAdminister medications via intravenous, oral, topical, subcutaneous, intramuscular, gastrostomy and other route of administrations within scope of practiceAdminister enteral feedings via nasal gastric tube or gastrotomy tubePerform bedside care such as wound dressing, changing bandages, inserting catheters, and giving injectionsObtain resident's vital signs such as respiration, temperature, blood pressure, and heart rate as needed and documentedSupervise certified nursing assistants and other auxiliary staffTaking verbal orders from the provider / physician and document appropriatelyNotifies physician of any pharmacist consultant’s recommendation for revision of medications, medication alerts, or changes in medication administration times as written in the Electronic Pharmacist Information Consultant (EPIC) form then implements the approved changesInitiate and / or discontinue peripheral IV heplockCall pharmacy for ordering prescriptions when discharging residents homeProvides hand-off report to receiving unit when transferring patientsMaintains a safe and comfortable environment for residents, visitors, and staffComplies with policies and procedures concerning infection control and safety for all individuals on the unitMaintains and develops competence through required in-service and continuing educational programsMaintains current CPR and IV certificationsDischarge patients to home or other healthcare settings as supervised by a registered nurseActive and contributing member of the Quality Assurance Performance Improvement CommitteeWhat We're Looking For
Full-time, Night shift, 12-hour shift. Rotating Weekend
Active NJ LPN license and BLS certified.Preferably a minimum of 2 years’ experience in post-acute care settings (i.e. skilled nursing facility, rehabilitation, long-term care).What We Offer
