SUMMARYThis professional will provide support services and short term rental assistance to homeless individuals, who have been released from incarceration within the last year, to secure and maintain stable housing and household income.

JOB DUTIES AND RESPONSIBILITIES

Provide all aspects of Housing Case Management for a case load in accordance with VOA policies and HUD and community standards.

Responsible for completing all HUD funding requirements including but not limited to housing inspections and rent reasonable certifications at entry and annually.

Collect documentation, calculate income, and complete rental calculations monthly for all active participants.

Schedule monthly, or more often as needed, meetings with participants including detailed home visits.

Receive and respond to referrals within 48 hours.

Schedule intake, confirm eligibility and complete enrollment within two weeks

Develop an individualized service plan for new participants.

Complete and update assessment, acuity scale and individualized service plan on biannual basis.

Responsible for ensuring participants quickly locate and maintain stable, affordable housing.

Create and maintain a good working relationship with landlords.

Responsible for ensuring participants obtain, maintain or increase income.

Responsible for ensuring participants minimize barriers impeding them to successfully take over rental payments within the time frame allotted.

Responsible for ensuring participants achieve employment and an income which supports their rent.

Provide on-going and relevant pro-social supports and life skills training.

Use evidence-based programming to help participants address challenges to stability.

Provide transportation to participants as needed.

Establish good working relationships with related agencies to maintain a comprehensive referral network for participants.

Maintain accurate records for all participants in accordance with VOA policies and relevant regulatory and funding sources, including but not limited to use of HMIS.

Prepare reports as requested.

Other duties as assigned.

EXPERIENCE AND EDUCATION QUALIFICATIONS

Associates Degree in Human Services, Social Work or related acumen and 3 years relevant experience or an equivalent combination of education, employment and life experience.

Bachelors degree and at least one year experience preferred.

Clean NYS Drivers License

Proof of COVID 19 Vaccination or approved exemption

Availability to occasional work evenings and weekends as required

Strong organizational skills.

Working knowledge of MS Office products

Flexibility required to respond to emergency participant needs
