Our team is looking for ahighly motivatedandexperiencedPediatric Nurseto joinour small,fiercely dedicatedgroupfor the position ofthe RN Patient Navigator. The Nurse Navigator role will support children with complex medical needs as well as their families by addressing barriers that prevent access to care. The Patient Navigator will understand the clinical pathways and direct the patients to health care services within the organization, at outside facilities, and within the community for timely diagnosis and treatment. Though a combination of telephone triage, care coordination, and collaboration across the healthcare system, the Patient Navigator plays a pivotal role ensuring that this special population of children have the support that is needed and deserved.

Daily tasks may include rooming patients, telephone triage, after visit follow up and tasks as provider directed, answering and appropriately routing phone calls, scheduling patient appointments, and various admin tasks to help the larger team.

The Nurse Navigatormustencompass strongleadership skills,organizational abilities, effective communication, time management, experience and proficiencyinEMRs,computersoftwareand applications. Attention to detail, problem-solving capabilitiesboth independently and in a group, and adaptability are essential for managingdailytasks efficiently. Additionally,theindividual must have stronginterpersonal skillsthatfoster collaborationwith aproactiveand positiveattitude.

Summary :

The Registered Nurse - Patient Navigator - MHMG MMCP role guides patients through the care continuum, establishing contact with recurrent and newly diagnosed cancer patients and / or family members, or caregivers. The Patient Navigator will understand the clinical pathways established by the Oncology Program and will direct the patients to health care services within the organization, at outside facilities, and within the community for timely diagnosis, treatment and survivorship.
