A company is looking for a Care Coordinator 1 to serve as a patient advocate and point of contact for care recommendations.
Key Responsibilities
Support patient experience preadmission and throughout the continuum of care, including coordinating admissions and providing program information
Ensure timely follow-up with patients and families, maintaining open communication
Facilitate internal transfers between sites and develop working relationships with interdisciplinary team members
Required Qualifications, Training, and Education
High School diploma required; Bachelor's degree preferred
Two years of customer service and relevant experience required, preferably in a healthcare setting
Experience with computers and proficiency in software platforms and EMRs
Knowledge of medical / psychiatric / chemical dependency diagnosis and treatment processes preferred
Valid driver license preferred for certain locations, with insurable status required
