Position Summary :
The primary function of the Case Manager is to oversee all aspects of case management, including interviewing families, assessing needs, determining risk categories, making referrals, implementing appropriate plans of care and planning follow-up services
Position Qualifications :
- Bachelor's degree or experience in social work, psychology, sociology, counseling, or other related field
- Experience in substance abuse care and service
- Understanding of medical and psychosocial issues related to opioid use and substance abuse disorders
Position Duties :
Accept newly-assigned cases after a licensed provider has completed diagnostic intake, and assess patient's strengths / weaknesses, examine solutions and establish immediate and long-term goals to collaboratively develop an individualized case management plan.Develop and maintain working relationships with community agencies to assist with plan management and goal achievement, including wraparound services, inpatient treatment centers, outpatient treatment groups, legal teams, government agencies, managed care, referral sources, educational systems and probation departments.Continually monitor and evaluate each patient's progression and assist in obtaining necessary resources to achieve goals.Provide ongoing face-to-face reviews with patients on a regular basis to ensure continuum of care needs are being met.Ensure plans are kept updated by maintaining accurate documentation of service objectives, outcomes, and corrective action revisions.Prepare case management reports for management and administrator, as requested.Actively participate in patient- and program-related meetings and trainings to enhance service continuity and effectiveness.Comply with documentation requirements as outlined in organizational policies and procedures and routinely verify patient files, reports and case histories are accurate, organized and updated.Collaborate with other staff members, community agencies and family membersWork to uphold organizational values and beliefs, using evidenced-based practices when delivering services to patients.Participate in the review of clinical issues, policies and procedures.Attend at least 2 hours of Clinical Oversight a monthEnsure patient rights and privacy are protected and confidentiality upheld according to organizational policies and procedures, state law and federal regulationsAs part of our commitment to maintaining a safe and productive work environment, Sanctuary Recovery Centers conducts background checks and drug screenings for all potential employees.
