About The Position

Experience : Minimum 1 full year Adult Home Health Experience or 1 full year Hospice Experience depending on the specialty

License : Must have license in hand

Must-Have : Recent specialty experience

Description :

Provides, coordinates, and directs the provision of home nursing care according to physician's orders.

Works based on agency policies and procedures through the competent application of the nursing process.

Responsible to the Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Home visits and periodic office meetings are required.

Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Average number of patients per day : 5-8.

Contractor will be assigned between 5-10 patients per day.

This equates to roughly 6-8 points per day.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.

Willingness to travel up to 75 miles is required.

Mileage will be reimbursed at the IRS standard rate.

Accepts local candidates at the same rate.

All professionals must be proficient with HomeCare HomeBase and have used it within the last 6 months.

If not, they will be required to complete a retraining course.

Must be proficient in OASIS as well.

The contractor's time will be approved weekly by designated personnel.

Weekly productivity expectations are 25-30 units.

Documentation must be completed within 24 hours of the appointment with the patient.

All documentation that is delayed or incomplete may impact billing and follow-up patient care.

It is important to accept every patient on the schedule regardless of location.

There are no guaranteed hours with this contract.

The contractor must achieve weekly productivity points in order to fulfill financial needs.

If not enough patients are available, floating to another clinic may be an option.
