A Brief Overview

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves a resource to others in work-team and department.

Position Details / Preferred Qualifications

At least one year of experience is strongly preferred. Previous work with OB patients is also highly preferred. This position provides social work services for Macdonald Women's hospital beds located at our Geauga Medical Center in Chardon. The role performs psychosocial assessments, supportive counseling, crisis intervention, and provides resource referrals and resources. This role is essential as it impacts timely and safe discharges, as well as patient and provider satisfaction. The schedule is Monday-Friday, 8 : 00am-12 : 00pm.
