Office Location : Pueblo
Coverage area : Pueblo County
Schedule : PRN, 15-20 visits / week available

As a Home Health Physical Therapist, You will :
Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipmentEstablish measurable goals and develop treatment plans to move patients toward their maximum level of independence and / or functionMaintain communication among the treatment team, physician, patient, and family members as applicableMaintain thorough and timely documentation of services provided to ensure proper billingSupervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice actQualifications
Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy programNew physical therapy graduates may be considered in select markets based on program availabilityAt least (2) years of appropriate experience as a physical therapist. Community / hospice experience is preferredValid license form the state of practicePossess and maintains current CPR certification
