Acute Care Social Worker - Day Shift + Relocation Support

A well-established healthcare system in North Florida is seeking a compassionate and experienced Acute Care Social Worker to join their multidisciplinary team. You'll play a vital role supporting patients through discharge planning, care coordination, and connecting them with community resources in a fast-paced, patient-centered hospital environment.

Position Overview

This full-time role focuses on acute care discharge planning, collaborating closely with medical teams to ensure seamless transitions for patients. Ideal candidates have recent acute care social work experience or substantial hospital social work background combined with LTAC or hospice experience. Experience strictly in psychiatric or community therapy settings without discharge planning will not be considered.

Work Schedule :

Full-time day shifts with occasional evening or weekend coverage as needed to support patient care.

Provide discharge planning and care coordination for acute care patients

Job Requirements

Licensed or eligible for licensure as a Social Worker in Florida

1+ year recent (within 3-4 years) acute care hospital social work experience preferred

Discharge planning experience is required

Consideration given to candidates with prior hospital social work experience even if not recent, and those with LTAC or hospice experience combined with hospital work

Strong interpersonal and communication skills with ability to work in a fast-paced healthcare environment
