Summary

Position Summary

Under the supervision of the Registered Nurse and Director of Nursing, the Licensed Practical Nurse (LPN) provides skilled nursing care through the assessment, planning, and implementation of individualized care plans. The LPN ensures residents’ needs are met while maintaining high standards of safety and quality. This role also supports daily unit operations and may assume leadership responsibilities as needed.

Key Responsibilities

Conduct ongoing patient assessments and gather relevant data to support individualized care.

Obtain patient history, perform focused physical assessments, and collect admission information per policy.

Observe and report changes in patient condition and diagnostic results to appropriate team members.

Deliver direct patient care in alignment with care plans and nursing standards.

Identify and document patient and family spiritual and cultural needs.

Collaborate with interdisciplinary team members to promote quality outcomes and resident satisfaction.

Qualifications

Experience : Two years of nursing experience required.

Education : High school diploma required; Associate’s Degree in Nursing preferred.

Licensure / Certification :

Current Maryland LPN license (or eligibility to obtain).

Basic Life Support (BLS) certification required.
