Job Overview
The Senior Community Care (SCC) program is a longitudinal, integrated care delivery initiative designed to coordinate clinical services for members in a facility setting. The Advanced Practice Clinician (APC) in SCC provides care to high-risk health plan members as part of an interdisciplinary team.

Primary Responsibilities
Conduct comprehensive patient assessments
Manage acute and chronic medical and behavioral conditions effectively
Ensure accurate and complete documentation, including ICD-10 coding
Collaborate and communicate with the interdisciplinary care team
Lead advanced illness and advanced care planning discussions
Provide counseling and education to patients and caregivers
Required Qualifications
Active, unrestricted state licensure (or ability to obtain before start date)
Education :
NP : Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program
PA : Graduate of an accredited Physician Assistant program
Certification :
NP : Board-certified by AANP or ANCC, with specialization in :
Family Nurse Practitioner (FNP)
Adult Nurse Practitioner (ANP)
Gerontology Nurse Practitioner (GNP)
Adult-Gerontology Acute Care Nurse Practitioner (AGACNP)
PA : Board-certified by the National Commission on Certification of Physician Assistants (NCCPA)
Active DEA licensure and prescriptive authority
Physical ability to perform job functions, including lifting 30 lbs, navigating stairs, and performing physical assessments
Reliable transportation
Ability to obtain a collaborative practice agreement, if applicable in the state
COVID-19 vaccination required (UnitedHealth Group will adhere to all relevant regulations and obtain necessary proof before employment)
Preferred Qualifications
1+ year of clinical practice experience (long-term care preferred)
Expertise in managing complex behavioral, social, or functional patient needs
Strong understanding of geriatrics, chronic illness, and advanced care planning
Proficiency in electronic medical records (EMR) documentation

Shift Hours : 8 AM – 5 PM, Monday – Friday
Dress Code : Medical attire (no specific scrub color required)
