Our patients receive the nation's best in digestive care, right here at home!\n\nAbout the Job : \n\nAssist provider as necessary; room patients, take vitals, triage patients, call in scripts, and other associated responsibilities.\n\nWhat You'll Do : \n

• Carry out the nursing process in person, by telephone (telephone triage), and via telehealth modalities\n
• Assess and identify patient needs, plan to achieve appropriate outcomes, intervene, and evaluate progress toward goals of care.\n
• Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals.\n
• Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state.\n
• Educates patient on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).\n\nWho You Are : \n
• Current NH Registered Nursing (RN) licensure is required.\n
• Certifications : Current BLS / CPR training.\n
• Prior work experience in a healthcare setting is required.\n
