We’re hiring a Licensed Practical Nurse (LPN) Case Manager to support Principal Care Management (PCM) and Chronic Care Management (CCM) programs for patients with chronic and neurological conditions.

In this hybrid Bay Area role, you’ll coordinate patient care, conduct follow-ups, and ensure high-quality, proactive support between visits. Working closely with Registered Nurses (RNs) and physicians, you’ll help patients stay engaged, compliant, and supported across care transitions.

Licensed Practical Nurse (LPN) Case Manager Responsibilities

• Deliver ongoing care coordination and patient follow-up for PCM / CCM programs.
• Conduct telehealth or phone check-ins to assess progress and reinforce care plans.
• Update and document care plans per Medicare PCM / CCM requirements.
• Track time-based activities for compliant CPT billing and reporting.
• Act as a liaison between patients, caregivers, and healthcare providers.
• Identify and escalate clinical issues to supervising RNs or medical staff.
• Support scheduling, documentation, and data management tasks.
