Join our team and make a positive impact on the lives of others! We are looking for a Licensed Social Worker (LSW) for our team in Ohio.

This is a part time, hybrid role with required fieldwork and a commute to our FreedomCare office in Cleveland, OH at least two times a month.

Department & Position Overview :

The Licensed Social Worker role will be essential to our Structured Family Caregiving (SFC) program, within the state of Ohio.

SFC is a caregiving arrangement in which a patient lives with a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient's independence in a home environment that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs while preserving the dignity, self-respect, and privacy of the patient.

As the Licensed Social Worker, you will be responsible for oversight of our patients' care by providing continued support to both the patient and caregiver; serving as both the escalation point and advocate for quality of service with FreedomCare and the SFC program.

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $26.00 and $33.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to : skill set, years of relevant experience, education, location, and licensure / certifications.
