Mental Health Worker

Title : Mental Health Worker (2nd Shift)

Duration : Long Term Contract

Location : Concord, NH

Duration of the Contract : Ongoing

Possibility for Extension : Yes, based on performance and client need

Work Location : Onsite – Concord, NH (Local NH candidates only)

DAILY DUTIES / RESPONSIBILITIES :

• Provide direct patient care and support under the supervision of nursing staff.
• Monitor patient behavior and report observations to the care team.
• Assist in maintaining a safe, therapeutic environment for patients and staff.
• Participate in the implementation of individualized care plans.
• Assist with patient transport, ADLs, and crisis intervention when needed.
