Overview

Seeking a Part-Time Physical Therapist to join the team at Colony Center Health & Rehab, Abington, MA.

Amazing therapy team to work beside & supportive Administrator!

The primary responsibility of the Physical Therapist (PT) is to plan, delegate and provide safe and competent physical therapy interventions as ordered by physician. In this role, you are entrusted to ensure that the highest degree of quality care can be provided to our residents at all times.

Responsibilities

• Conduct thorough evaluations of patients’ physical abilities and limitations.
• Develop individualized care plans based on patient assessments and goals.
• Implement treatment programs that may include exercises, manual therapy, and modalities.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care.
• Educate patients and their families about treatment options, home exercises, and injury prevention.
• Maintain accurate documentation of patient care activities and outcomes.
• Participate in discharge planning to facilitate smooth transitions from inpatient to outpatient care.
• Stay updated on best practices in physical therapy, including advancements in orthopedics and acute care.
