Fathers To Founders is looking for an Enhanced Care Management Director. The Enhanced Care Management Director will be responsible for the operational, administrative, and clinical oversight of FTF’s Enhanced Care Management (ECM) programs. This position requires a strong understanding of Medi-Cal ECM guidelines, care management workflows, and leadership skills. The Director should have experience in building and maintaining relationships within healthcare systems, including managed care plans, clinics, hospitals, and behavioral health providers. Please note that this is not a virtual position; the Director will regularly work in San Francisco and Sacramento. Proficiency in Salesforce is required.

Fathers To Founders (FTF) is a California-based social enterprise advancing community stabilization through healthcare navigation, care coordination, and workforce development. As an Enhanced Care Management (ECM) and Community Support provider, FTF partners with managed care plans, public agencies, and community organizations to improve outcomes for individuals experiencing chronic medical, behavioral health, housing, and social barriers.
Responsibilities
Lead day-to-day oversight of ECM member services, care planning, and interdisciplinary team coordination.
Ensure care delivery and documentation meet all DHCS ECM standards and managed care plan requirements.
Train, coach, and supervise Care Managers in trauma-informed engagement, motivational interviewing, and complex case navigation.
Conduct case reviews, escalations, and quality improvement initiatives to improve service consistency and member outcomes.
Manage workflows, productivity expectations, caseload distribution, scheduling, and documentation quality in Salesforce.
Track and report program performance metrics including referrals, utilization, engagement rates, and outcomes.
Serve as the primary operational liaison for managed care plans, participating in reviews, audits, reporting, and performance meetings.
Build and maintain strong working relationships with hospitals, primary care providers, behavioral health agencies, housing networks, and community partners.
Collaborate with the Executive Director on service expansion, staffing strategy, and sustainable growth planning.
Qualifications
Minimum 5 years experience in care coordination, community health, behavioral health, medical social work, or related field.
Minimum 3 years supervisory or program management experience.
Experience navigating healthcare relationships such as managed care plans, hospital systems, health centers, or provider networks.
Salesforce proficiency required (ECM or care management environment strongly preferred).
Strong working knowledge of Medi-Cal Enhanced Care Management standards and documentation expectations.
Ability to work on-site and in the field, with regular travel between San Francisco and Sacramento.
Clear communication, conflict de-escalation, and stakeholder engagement skills.
Preferred Qualifications
Licensed or license-eligible clinician (LCSW, LMFT, LPCC, RN) or master’s degree in Social Work, Public Health, Healthcare Administration, or related field.
Experience implementing or scaling care coordination programs.
Experience supporting high-acuity populations such as homelessness, reentry, substance use, or chronic medical / behavioral health conditions.
