Requirements:

• Must hold active LPN/LVN license in practicing state.
• LTC/SNF/Assisted Living experience preferred.
• Strong clinical, communication, and care skills.
• Able to follow care plans and collaborate with staff.

Responsibilities:

• Clinical Care:
  • Assist with ADLs and monitor resident health status.
  • Administer medications and treatments.
  • Document resident care and report all changes.

• Collaboration & Safety:
  • Follow physician orders and established care plans.
  • Promote a safe, clean, and supportive environment.
  • Provide emotional support and companionship.
