Advanced Practice Provider ( NP or PA)

Advanced Practice Provider ( NP or PA). Experience is preferred but not necessary. The ideal candidate should be located in the Raynham, MA vicinity.

We are a group of highly motivated healthcare providers that include MDs, NPs, and PAs. We combine 20+ years of experience in internal medicine, geriatrics, hospital medicine, and hospice care. We believe in the use of technology to deliver efficient and amazing care for our patients.

Position Summary :

To take care of patients in the nursing home setting. Long-term care and skilled patients. Working with an MD, diagnosing, treating, and coordinating the care of these patients. We are looking for a full-time, highly motivated and, caring provider who's passionate about delivering the highest level of care possible for their patients. The coverage is mainly Monday- Friday position.

Requirements :

• 1+ years of professional experience
• Experience using electronic health record (EHR) software
• Ability to learn / stay abreast of relevant practices
• Critical thinking is needed when issues or concerns arise with patients and families of patients
• Detail-oriented, organized, and with a high degree of accuracy
