Qualifications

Active LPN License : Current and valid with the Arizona State Board of Nursing.

Home Health Experience : Required; WellSky (Kinnser) experience preferred.

Driver’s License : Valid Arizona driver’s license required.

Reliable Transportation : Must have dependable vehicle and auto insurance.

CPR Certification : Current and valid.

TB Clearance : Current health card or ability to obtain one per federal and state guidelines.

Education : Graduate of an accredited Licensed Practical Nurse (LPN) program.
