Job description

We are hiring a Clinical Pharmacist to join our team! We're seeking motivated, patient-focused professionals with experience in infusion therapy , ready to make a difference in home infusion patient care.

About the Role :

Seeking a Clinical Pharmacist with experience in infusion therapy —either in a hospital or home infusion setting. This position requires experience in reviewing, managing, and dosing compounding products , particularly for immunoglobulin (IG) and chronic therapy patients, and supporting sterile compounding operations . The ideal candidate will have a strong understanding of assessing patient suitability for home infusion, designing individualized care plans, and collaborating with interdisciplinary teams .

Location : Louisville, KY

Shift : Standard business hours; Rotating schedule for all pharmacists (every 8th Saturday) : Weekday shifts : 7am–3pm, 9am–5pm, 11am–7pm; Saturday shift : 9am–5pm

Pay : $46.69 - $77.84 / hr

Key Responsibilities :

  • Assess patient suitability for home infusion therapy.
  • Design, monitor, and evaluate individualized pharmacy care plans.
  • Recommend or adjust medication therapies based on clinical data.
  • Prepare, compound, label, and dispense medications per USP and DEA standards.
  • Conduct nutritional assessments and provide patient / caregiver education.
  • Collaborate with nursing and other departments to ensure continuity of care.
  • Monitor controlled substance inventory and report discrepancies.
  • Provide drug information to healthcare professionals and patients.
  • Participate in CEU and professional competency programs.

Qualifications :

  • Education : Graduate of an accredited School of Pharmacy .
  • License : Active Pharmacy License in the State of Kentucky.
  • Experience : One of the following :
  • 1+ year experience in sterile IV admixture procedures
  • Knowledge of non-sterile compounding, immunizations, and MTM
  • Completion of ASHP-accredited Pharmacy Residency Program
  • Completion of an Advanced Pharmacy Practice Experience
  • Equivalent experience approved by VP / SVP of Pharmacy
  • Technical Skills : Proficient in Microsoft Word, Excel, and PowerPoint .
  • Attributes : Strong customer service and patient-care orientation .
