Washington Licensed Behavioral Health Navigator

A company is looking for a Behavioral Health Navigator SW.

Key Responsibilities

Conduct mental health assessments and safety planning for emergency department patients

Create outpatient plans and secure appointments prior to discharge or within 48 hours

Provide active case management for behavioral health needs for 90 days post-discharge

Required Qualifications, Training, and Education

Master's Degree in Social Work (MSW)

Licensed in the state of Washington as a Licensed Social Worker Associate & Independent Clinical (LSWAIC)

Health care industry knowledge preferred

Behavioral health experience / knowledge preferred
