About The Position

Job Title : Physical Therapist – Home Health

Location : Stockton, CA (Central Valley)

Assignment Length : 13 Weeks

Hours : 40 hours / week (based on completed visits)

Shift : Day shift (Tues-Sat or Wed-Sun)

Position Type : Field / Home Health

Job Details :

We are seeking a motivated Physical Therapist for a 13-week home health assignment in Stockton, CA, serving the Central Valley region. This field position offers a flexible schedule and the chance to provide high-quality care directly in patients’ homes.

Schedule Options :

Tuesday through Saturday

OR Wednesday through Sunday

Key Requirements :

CA Physical Therapy License preferred (Candidates with pending CA license are welcome if they have applied prior to submission)

Home Health experience preferred , but candidates without experience will be considered

OASIS documentation experience is required

COVID exemptions are permitted

Must be willing to travel within the coverage territory (mileage reimbursed at IRS standard rate)

Workload & Hours :

Guaranteed 40 hours per week if 25 or more visits are completed

Hours calculated based on completed patient visits (25 visits = 40 hours)

Rate may vary depending on coverage territory and candidate experience

Local candidates are welcome to apply
