Travel Physical Therapist (PT) – Rehab

Location : Potosi, MO

Hours / Day : 8

Shift Notes : M-F, 8-hour days. Start time flexible as long as consistent and communicated with DOR (must be before 6 AM and no later than 10 AM). 35 GWW.

Position Overview

Provide physical therapy evaluation, treatment, and rehabilitation services to patients with physical functioning disorders and / or wound care needs. Develop individualized plans of care and utilize evidence-based practice to support patient progress in accordance with regulatory and clinical practice standards.

Essential Functions

Evaluate and rehabilitate patients with physical functioning disorders and / or wound care needs.

Utilize standardized assessments and evidence-based practice to guide interventions.

Administer procedures including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices.

Assess patient progress and adjust treatments accordingly in clinical records.

Evaluate treatment outcomes and modify services based on patient evaluation and referrals.

Provide consultation and education to patients, families, caregivers, and other healthcare providers.

Communicate effectively with supervisors and interdisciplinary team regarding patient progress and treatment plans.

Prepare comprehensive discharge summaries in compliance with regulatory and clinical standards.

Document all required information in the electronic documentation system and maintain accurate patient records.

Record treatment minutes in accordance with regulatory and clinical requirements.

Participate in patient care conferences, utilization review meetings, and rehabilitation team meetings.

Participate in facility and company-required trainings, in-services, and conferences.

Obtain necessary DME or supplies in compliance with regulatory and clinical practice requirements.

Supervise and support physical therapist assistants, rehabilitation aides, and / or students.

Adhere to APTA and state-required practice acts, code of conduct, and ethics during professional practice.

Submission Requirements

1 year of PT experience – Required

LTC or Rehab experience – Required

MO State License – Required

New Grads – Accepted if Level II experience is completed (Level I observation alone is not sufficient)
