Job Summary :

We are seeking a compassionate and detail-oriented Registered Nurse (RN) to conduct in-home assessments for our clients. The RN will evaluate each patient’s health status, home environment, and care needs to ensure appropriate care plans and services are in place. This role is ideal for nurses seeking flexible hours and meaningful one-on-one patient interaction.

Responsibilities :

Perform comprehensive in-home nursing assessments to determine patient health status and care needs

Document findings accurately and submit reports in a timely manner

Communicate assessment results and recommendations to the care coordination team

Monitor for safety hazards, environmental risks, and overall suitability of the home setting

Educate patients and families on health management, medications, and care instructions as needed

Maintain compliance with all agency policies, HIPAA, and state regulations

Collaborate with interdisciplinary teams to support patient care plans

Qualifications :

Current and valid RN license in the state of [Insert State]

Minimum of 1 year of clinical nursing experience (home health experience preferred)

Strong assessment and critical thinking skills

Excellent communication and documentation abilities

Reliable transportation and proof of valid driver’s license and insurance

Schedule :

PRN / As Needed – flexible scheduling based on availability and patient demand

Compensation :

Competitive hourly pay

Mileage reimbursement for all travel
