Positon : Physical Therapist

Location : Brooklyn, NY

Pay Rate : $54 / hr

Schedule : 5x8

Responsibilities :

• Evaluation, treatment planning and delivery of skilled PT services to clients of this facility.
• PT is responsible for the timely initiation of services.
• PT is responsible for screening, evaluation and treatment, treatment planning, goal setting, family / caregiver education.
• PT will document all services provided to ensure accurate and timely billing of therapy services.
• The PT assures that patient care follows the organization's guidelines; meets professional standards, community needs and is in compliance with Federal and State guidelines.
• The PT serves as a patient and rehabilitation advocate for current and prospective patients and to internal and external customers. Ensures cross-disciplinary coordination in order to provide consistent patient care. Participates in quality assurance and customer service activities.
