MedAdventures is seeking a skilled and compassionate Registered Nurse (RN) to join our Home Health team in Hermitage, PA . This is a meaningful opportunity to provide essential nursing care to patients in their homes, helping them maintain independence and quality of life in the community.

As a Home Health RN , you’ll deliver high-quality skilled nursing care, complete comprehensive assessments, and ensure documentation accuracy in accordance with agency policies and state / federal regulations. This is a field-based role requiring clinical visits within the designated coverage area.

    • Key Responsibilities

  • Provide skilled nursing care and coordinate patient services per individualized plan of care
  • Complete OASIS assessments accurately and in a timely manner
  • Utilize Homecare Homebase (HCHB) for documentation and visit management
  • Monitor and report changes in patient condition to the care team and physician
  • Administer medications and treatments as prescribed
  • Educate patients and caregivers on disease processes, medications, and home safety
  • Participate in case conferences, quality reviews, and performance improvement initiatives
  • Maintain compliance with all agency, state, and federal requirements

    • Qualifications (Required)

  • Active Pennsylvania RN license
  • Current BLS certification (American Heart Association)
  • Minimum 1 year of Home Health experience
  • Proficiency with OASIS and Homecare Homebase (HCHB)
  • Reliable transportation, valid driver’s license, and proof of auto insurance

    • Preferred

  • 2+ years of home health or post-acute nursing experience
  • Strong clinical assessment and critical thinking skills
  • Experience providing patient and family education
