Care Coordinator
Schedule : Monday–Friday, 8a–5p (minimal evenings / weekends)
Focus : Completing assessments and initiating care plans for a high volume of new members.
Contract Duration : 6 months (with possiblity of extension / conversion)

Summary :
The Care Coordinator supports members by completing health and psychosocial assessments, developing initial care plans, and coordinating services with an inter-disciplinary care team (ICT). The role centers on telephonic / electronic outreach and ensuring members receive timely, culturally competent support.
Key Responsibilities :
- Conduct health and psychosocial assessments.
- Develop and document individualized care plans with the ICT.
- Identify barriers to care and implement interventions.
- Educate members / caregivers on resources and benefits.
- Coordinate with facility and community-based care managers to ensure smooth transitions and avoid duplication of services.
- Maintain accurate, timely documentation and share process improvement ideas.
Requirements :
Diploma, Associate’s, or Bachelor’s in healthcare-related field or equivalent experience.3+ years in care coordination, case management, discharge planning, or related healthcare role.Strong communication, organization, and documentation skills.Intermediate MS Office proficiency.Knowledge of healthcare regulations and community resources.Work Environment :
Primarily office or remote setting.Standard business hours; minimal weekend work expected.
