Position Summary :

We are hiring a field-based Nurse Practitioner (AGNP or CNP) who thrives in an independent, flexible environment. If you're not yet certified in wound care, we'll pay for it. This is a great fit for someone looking to grow their clinical specialty while making a direct impact on a vulnerable population.

Key Responsibilities :

• Visit assisted living, memory care, and senior communities to provide primary care
• Address wounds, chronic conditions, and preventative care in place
• Partner with care teams and families to ensure holistic support
• Keep accurate records and communicate clearly with all stakeholders
