Qualifications Graduate of a vocational / technical nursing program
Current LVN / LPN license in the state of Texas Knowledge of lab
procedures, diagnostic testing, and EMR systems Strong
documentation, communication, and organizational skills Key
Responsibilities Review medical records, identify gaps in care, and
schedule preventive services per physician-approved protocols
Collaborate with providers, Nurse Navigators, and families to
ensure continuity of care and patient support Educate patients
using evidence-based guidelines to promote engagement and health
