Job Description

Job Title : Registered Nurse

Job Specialty : Home Health

Shift : Day Shift, 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : Not guaranteed

Experience : Minimum of 1 full year of Adult Home Health or Hospice experience, depending on the specialty

License : Must have a license in hand

Certifications : CPR, BLS

Must-Have :

• Proficiency with HomeCare HomeBase (HCHB) and OASIS, used within the last 6 months or completion of a retraining course during compliance
• Reliable transportation, driver's license, and auto insurance
• Willingness to travel up to 75 miles from the home clinic
• Ability to meet productivity points of 30 per week
