The Manager-Case Management is responsible for managing and supervising the team tasked with working collaboratively with various stakeholders to provide assessment planning facilitation care coordination evaluation and advocacy for options and services to meet a patient's comprehensive health needs. HSHS leaders are tasked with leading a specific area or function and furthering the mission and core values of the organization. In the Franciscan tradition HSHS leaders put the needs of others before their own and model HSHS core values holding themselves and others accountable to these values.
Education
Bachelor's degree in Nursing or Social Work is required.
Master's degree in Nursing or Social Work is preferred.
In lieu of bachelor's degree diploma or an associate's degree in nursing or in relevant clinical field with a minimum of 3 years of case management experience may be considered.
Experience
3 years leadership experience in a progressively responsible role is required.
2 years of case management experience is preferred.
Certifications Licenses and Registrations
Licensed Registered Nurse or Social Worker in the state of practice is required (IL or WI).Case Management certification (ACM CCM RN-BC) is preferred.
In lieu of licensure in Nursing or Social Work may consider candidates with other clinical credentials.
