Position Summary
The Registered Nurse (RN) will provide skilled nursing care and case management in patients’ homes under physician orders. The RN is responsible for assessing patient health, implementing and evaluating care plans, educating patients and families, coordinating with multidisciplinary team members, and ensuring compliance with regulatory and agency standards. The RN will often work independently and must ensure safety, quality, and compassion in care delivery.
Key Responsibilities
Conduct comprehensive nursing assessments on admission and at regular intervals : physical, psychosocial, environmental.
Develop, implement, and update individualized care plans in collaboration with physicians, therapists, aides, social workers, and family.
Administer medications, treatments, and therapies as ordered (e.g., wound care, IV, injections) and document fully.
Monitor patient vital signs, condition changes, and response to treatment. Report findings and adjust care plan as needed.
Provide patient / family education : disease processes, medication management, wound care, safety in the home, etc.
Coordinate care : collaborate with therapists (PT, OT), social work, home health aides, and other providers to ensure an integrated approach.
Supervise, mentor, and support Home Health Aides and LPNs under the RN’s purview (as applicable).
Ensure all documentation is accurate, timely, in compliance with agency policy, payer requirements (Medicare / Medicaid), and regulatory standards.
Conduct regular home environment safety evaluations (infection control, fall risks, cleanliness, proper equipment usage).
Participate in quality assurance, audits, performance improvement activities. Track outcomes, incidents, and client satisfaction.
Ensure compliance with all legal, regulatory, and ethical standards (e.g. patient confidentiality, state licensing, mandatory training).
Qualifications
Valid, unencumbered Registered Nurse license in Pennsylvania.
Minimum of 1-2 years clinical nursing experience; prior home health or community health experience preferred.
Strong clinical skills : wounds, IVs, medication management; ability to assess, triage, and identify potential complications.
Excellent communication skills—able to interact well with patients, families, and multidisciplinary team.
Strong organizational skills; ability to manage a caseload, prioritize visits, travel between home sites.
Self-motivated, independent worker with good problem-solving ability.
Reliable transportation, valid driver’s license, and willingness to travel within assigned counties.
Up-to-date certifications : CPR, possibly first aid; any other required state / home health nursing credentials.
Ability to pass background check and required health screenings / immunizations.
Working Conditions
Primary work takes place in patient’s homes across Western & Central Pennsylvania. Office visits / travel may be required.
Variable schedule : visits may occur during weekdays, evenings, sometimes weekends. Caseloads and travel may lead to flexible hours.
Working alone in home environments; may encounter challenging home settings, variable cleanliness or layout, pets, etc.
Exposure to medical risk (bodily fluids, wounds, infection control situations); PPE and safety training provided.
