As the PM RN, you will combine hands-on clinical care with meaningful relationships, ensuring residents feel safe, heard, and genuinely cared for at the end of each day.

Schedule
PM / Evening Shift (exact hours can be customized; e.g., 2 : 00 p.m. – 10 : 30 p.m.)Rotating weekends and holidays as scheduledFull-time position with consistent hoursWhat You’ll Do
Provide comprehensive nursing care to residents, including assessments, medication administration, treatments, and monitoring of changes in conditionPerform focused and head-to-toe assessments and communicate timely with providers and families when status changesCoordinate resident care with caregivers, therapists, and other team members to ensure a smooth transition from day shift to night shiftOffer calm, reassuring support to residents and families during evening routines, including pain management, comfort care, and emotional supportComplete accurate and timely documentation, follow care plans, and support care conferences as neededProvide clinical leadership on the PM shift by supporting CNAs / care staff, answering clinical questions, and modeling safe, person-centered careChampion infection prevention, safety, and quality standards throughout the buildingWhat We’re Looking For
Current Registered Nurse (RN) license in the State of WisconsinPrevious experience in assisted living, long-term care, memory care, or similar setting preferred, but strong new RNs with solid clinical skills are welcomeStrong assessment, critical thinking, and prioritization skills—especially during busy PM hoursCompassionate communicator who can build trust with residents, families, and coworkersReliable, professional, and able to work independently while being a strong team playerWhat We Offer
