Home Health RN Registered Nurse - Empath Home Health - Port Charlotte, North Port, Venice

Empath Home Health, a member of Empath Health, is seeking a compassionate Home Health RN Registered Nurse to provide skilled nursing care for patients in their homes throughout Charlotte county (Port Charlotte, North Port, Venice, Englewood, Rotonda West, Arcadia, and surrounding areas). In this full-time role, you'll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.

Locations : Home health visits in Charlotte county including : Port Charlotte, North Port, Venice, Englewood, Rotonda West, Arcadia, and surrounding areas

Schedule : Full-Time, M-F 40 hours. Required weekend / holiday on-call rotations. PRN is available if desired!

What You'll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
