Description :

Assess for level of care for patients struggling with depression, psychosis, substance use, and other significant mental illnesses.

Provide biopsychosocial assessments, crisis intervention, short-term counseling, linkage with resources, and planning for transitions of care for patients and their families / significant others of all ages in any patient care setting.

Deliver psychoeducation and may facilitate support groups.

Consult on psychosocial aspects of care as a member of the interdisciplinary treatment team.

Serve as a liaison to community programs.

Assist and collaborate with multidisciplinary teams on discharge planning.

Practice independently while providing support to patients in adjusting to illness.

Facilitate goals of care conversations with patients and families.

May provide education to staff and other departments and participate in committees.
