Health Navigator

A company is looking for a Navigator I to provide support and education to stroke survivors and their care partners.

Key Responsibilities

Manage a caseload of stroke survivors and care partners, providing remote support through various communication channels

Assess educational and social needs, facilitating a smooth transition to home care post-hospitalization

Conduct onboarding interviews, assessments, and virtual support groups while collaborating with the clinical care team

Required Qualifications

Bachelor's degree or Health coaching certification (NBHWC) required

2-5 years of experience in a patient-facing role, preferably with post-stroke care

Case management experience is highly preferred

Proficiency in basic medical terminology and digital applications

Regular and reliable internet access for remote work and client engagement
