• What You'll Do :
• Conduct in-home visits to patients with chronic or life-limiting conditions to assess and treat symptoms and provide ongoing education of disease processes
• Collaborate closely with patients, families, and existing clinical team to create and manage individualized care plans
• Support advance care planning and goals-of-care discussions with empathy and clarity
• Serve as a clinical leader in the programs continued development and growth
• Advocate for patients needs across multiple care settings
• What Were Looking For :
• Current California Nurse Practitioner (NP) license
• Experience in palliative care, hospice, geriatrics, or home health preferred
• Excellent communication and patient relationship skills
• A proactive mindset with the ability to work both independently and as part of a collaborative team
• Passion for delivering compassionate, patient-first care in a home setting

Pay : $115,470.79 - $139,061.59 per year
