Registered Nurse Assessment Coordinator

Full job description {#jobDescriptionTitleHeading tabindex="-1"}\n\nThe Assessment Nurse (AN) assists in planning, implementing, and evaluating nursing care with\n\nthe Resident Assessment Instrument (RAI) coordinator. If the AN is a Registered Nurse, the AN\n\nmay be designated as RAI coordinator. The AN assists with the pre-admission screening\n\nprocess, admission process and nursing interventions. The AN is responsible for clinical\n\ndocumentation to include observing, recording, and reporting to the appropriate person the signs\n\nand symptoms that may be indicative of a change in the resident. The AN communicates with\n\nthe physician, nurses, nurse aides, interdisciplinary team members, residents and resident\n\nrepresentatives as to resident's condition or response to treatment. The AN follows and\n\ncompletes the RAI process including documentation requirements, Minimum Data Set (MDS),\n\nCare Area Assessment process, care plan development, and care plan implementation.\n\nThe Assessment Nurse is responsible to the Director of Nursing.\n\nJob Type : Full-time\n\nBenefits\n
