Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.
We are now hiring a Part-Time Advanced Nurse Practitioner in Bergen, NJ. The Nurse Practitioner collaborates closely with the Registered Nurse (RN), Psychiatrist, and other team members to ensure comprehensive assessments, effective care coordination, and consistent psychiatric support for youth experiencing target behaviors. This role is vital in addressing medical and psychiatric needs, reducing service delivery gaps, and supporting families. Providing face to face supports for 10 kids.
$100-$100 / per hour
Responsibilities
- Conduct comprehensive medical assessments to identify potential conditions contributing to target behaviors
- Collaborate with the RN and Psychiatrist to integrate findings into care plans.
- Serve as a liaison between the youth’s primary care and specialty providers to ensure seamless medical care
- Support families in managing medications and adhering to treatment plans
- Address challenges in service delivery, including family support and regional travel demands
- Partner with psychiatrists to provide consistent psychiatric care to youth in home and community settings
- Conduct home visits to monitor progress, adjust care plans, and ensure treatment continuity
- Facilitate virtual services alongside psychiatrists to mitigate travel burdens and maintain service quality
- Act as a key team member in developing and implementing care strategies tailored to the needs of youth and families
- Provide insights to address barriers and improve care delivery models
- Maintain accurate and timely medical records in accordance with organizational and regulatory standards
- Ensure compliance with all relevant healthcare guidelines and ethical practices
- All other duties as assigned
- Employee must cooperate with the licensee and department staff in any inspection, inquiry, or investigation
- Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing
Minimum Requirements
Master’s or Doctorate degree in Nursing with certification as a Nurse PractitionerCurrent and unrestricted NP license in the applicable state(s)Experience in pediatrics, psychiatry, or community health is highly preferredStrong skills in assessment, care coordination, and family engagementAbility to work effectively in home and community settings, including occasional virtual collaborationExcellent communication and interpersonal skillsClinical expertise in identifying and managing medical and psychiatric conditions in youthStrong organizational and problem-solving skills to navigate diverse care needsCompassionate and culturally sensitive approach to working with familiesFlexibility and adaptability in a dynamic service environmentWhy Community Options?
