License : VA or compact license required

Certifications : BLS

Must-Have :

• BSN required
• 3 years of critical care, acute care, or medical surgical RN experience
• EPIC charting experience required

Description :

The Hospital at Home Registered Nurse provides direct patient care to inpatients at the acute level within the comfort of the patient’s home.

Each patient has an individualized plan of care that determines the tasks and duties for home visits.

The Home Hospital RN is responsible for integrating inpatient care delivery into the patient’s living space.

This role utilizes technology and remote clinical care coordination.

The Home Hospital RN delivers quality care through the nursing process within a team nursing model.

They will document patient care and collaborate with a multi-disciplinary healthcare team.

The RN will provide therapeutic interventions in the home while maintaining high-quality and safety standards.

Candidates must have experience managing acute care or medical surgical adult and / or geriatric patient populations.

Telephone triage experience is preferred.

Home health experience is also preferred.

Rotating shifts will include days and evenings, including weekends and holidays.

Candidates should be prepared to float within scope if needed.
