American Traveler is seeking a Physical Therapist for a home health assignment requiring at least 1 year of home health experience and a valid FL or compact PT license.
\n \n Responsibilities\n
Home health setting covering Sarasota and Charlotte CountyPatient population includes med / surg, cardiac, ortho (hips, knees, shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trach, trauma, amputation, and ADL needsHome skilled visits consist of 6-8 visits per day with a minimum of 6 points per day; caseload may increase at manager discretionWork schedule is day shift, Monday through Friday from 8 : 00 to 17 : 001-2 weekend rotations per contract are requiredSelf-scheduling of visits and hours based on office needsUse of BayCare Forms, BayCare Scheduling, Ace Timesheet, and Cerner (preferred) chartingTravelers may float within the hospital or to affiliated hospitals as requiredResponsibilities include evaluating and treating a diverse adult patient population in the home settingPatient assessments focus on safety, injury prevention, and developing individualized exercise and recovery plansAll scheduling is managed by the therapist in collaboration with the officeLaptop, MiFi for internet, and portable printers are providedMust pass a pre-employment EKG exam with up to 3 attemptsPre-start and annual modules must be completed as part of onboarding and continuing complianceAny solid color scrubs may be wornReturn travelers must have completed a BayCare contract within the last year and meet specialty-specific rulesTravelers must live at least 100 miles from the facility to qualify for this roleSmoking is not allowed within the facility or on its grounds\n \n Requirements\n
