JOB SUMMARY : Professional member of the home health team who provides skilled nursing visits in
patient’s homes under the direction of plan of treatment established with physician and input from
patient / caregiver.
DUTIES AND RESPONSIBILITIES :
1. Maintain self in a professional manner at all times and situations
2. Maintain strict confidentiality of all patient, employee and Haven Specialty Home Care's information
3. Participate in all Haven Specialty Home Care's in-service programs
4. Provide initial and on-going assessment of client needs using the OASIS data elements incorporated
into the comprehensive assessment
5. Formulate and implement plan of cares with input from patient / caregiver and in conjunction with the
physician
6. Evaluate effectiveness of care plan and make necessary adjustments
7. Provide for the emotional and physical comfort and safety of client taking into consideration their
rights and cultural background
8. Receive and transcribe physician orders
9. Notify physician of unusual reactions and / or changes in client’s condition
10. Document all appropriate observations and treatments in keeping with Haven Specialty Home Care's
policies and procedures
11. Participate in case conferences, team meetings, staff meetings and performance improvement activities
as assigned
12. Provide any skilled nursing service for which appropriately trained which is prescribed under the plan
of care. Prepare clinical progress notes as per Haven Specialty Home Care's policies and procedures
13. Other duties assigned by supervisor and for which he / she is qualified
14. Adheres to all Haven Specialty Home Care's policies and procedures including but not limited to the
HIPAA privacy rule
Available Shifts :
Part Time Days - up to 16 hours per week
QUALIFICATIONS :
Pay Details :
Hourly Pay $50 - $54 per hour depending on experience
Ideal candidates will be graduates of an accredited nursing program and must hold a valid nursing license in the state of Washington. With over a year of experience under your belt, you’ll have the chance to work closely with patients and their families, providing support and care that's tailored to their unique needs.

What is the EEOICPA?
The EEOICPA (Energy Employees Occupational Illness Compensation Program Act) under the United States Department of Labor has a free (to eligible energy workers) program to completely cover the medical costs of energy workers such as uranium miners, millers, and haulers or beryllium workers from the Cold War era.
This program also provides monetary compensation for damages suffered. Illnesses include, but are not limited to radiation-induced cancers and beryllium-related illness.
