As a Case Manager, you will play a vital role in ensuring the well-being and quality of life for our residents. Responsibilities :
Job Responsibilities :
Maintain firm adherence to the resident’s “Bill of Rights”, as they relate to the facility
To communicate with the resident’s care providers
Assessments and coordination of admission / re-admission from the community, nursing homes and hospitals
Coordinate and supervise clinical care for residents
Maintain clear and concise documentation in accordance with facility requirements
Qualifications / Requirements :
Prior experience in Long Term Care is a PLUS or 1 year of experience as a Case Manager required
Previous Compuware experience is a plus
Excellent phone skills and ability to work with families / other care providers
Strong organizational skills, passion and compassion for the population
Ability to work cooperatively with multidisciplinary team
