Job Description :

Sunbelt Staffing is hiring an experienced Home Health RN for a travel position based in Uniontown, PA. This assignment offers autonomy, flexibility, and the opportunity to make a direct impact on patients lives in their own homes. Strong charting skills with Homecare Homebase and OASIS (24-hour turnaround required) are essential.

Key Responsibilities :

Complete in-home nursing visits and assessments

Submit accurate OASIS documentation within 24 hours

Use Homecare Homebase to manage patient records

Monitor conditions and update care plans accordingly

Educate patients and caregivers on care procedures and medications

Qualifications :

RN license valid in PA or compact states

At least 2 years of home health RN experience

Familiarity with HCHB and OASIS

Valid driver's license, insured vehicle, and dependable transportation

Must meet Joint Commission standards for onboarding, background, and drug screening
