Our client is seeking a dedicated Qualified Developmental Disabilities Professional (QDDP) to oversee the planning, coordination, and compliance-related supports for adults living in their licensed group homes. The QDDP will serve as the lead case manager for the 25-30 individuals living in the homes.
Full time with flexible hours within business needs.
SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
- Develop, update and monitor person-centered individual support plans (ISPs)
- Review and audit daily documentation
- Submit CHRIS incident reports
- Train and coach Direct Support Professionals (DSPs) and house managers
